Using PDF Forms
Before using the forms make sure you have the latest version of adobe reader.
Filling out the application forms
1. Determine your membership type by reading the descriptions beneath each category.
2. After you have determined your membership category, download the appropriate membership application by following the link titled “Application Form”.
3. Once the form has been downloaded you should have the capability of typing into the form by clicking into the text boxes located on top of each line.
4. After you are done completing the form please print off a copy and fax it to 410-263-1659 and put it to the attention of Amy Luckado, Membership director. You also have the option of saving the form to your desktop for your records. You may also email the completed form to Amy Luckado at aluckado@nahad.org with “Membership Application” as the subject.
